Getting the intent wrong produces a valid but wrong code
Poisoning and adverse effect are not interchangeable. An adverse effect is a reaction to a drug correctly prescribed and correctly taken; a poisoning is an overdose, a wrong substance, or a wrong route. Underdosing means taking less than prescribed.
